Why this applies to Pirfenidone

Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the gist for pirfenidone (Esbriet): your liver uses an enzyme called CYP1A2 to break it down. Lentinan, in lab studies, looks like it might rev that enzyme up. If that happened in people, your body could clear pirfenidone a bit faster, which might leave less of it in your system and could make it work less well for your lung scarring.

Pirfenidone is the active drug itself (not a prodrug that needs switching on), so speeding up its breakdown means weaker effect, not stronger. The good news is this is only seen in test-tube and animal research, never confirmed in humans, so I wouldn't panic. Still, because keeping steady drug levels matters here, please run any lentinan supplement past your pharmacist or doctor first.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
