1,3-dmba and Lidocaine

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Why this applies to Lidocaine

Lidocaine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmba interacts with.

Theoretically, 1,3-DMBA might modestly increase the levels of CYP2D6 substrates.
In vitro research shows that 1,3-DMBA weakly inhibits CYP2D6 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with 1,3-DMBA and lidocaine. Lidocaine isn't a prodrug, so it works as-is and doesn't need an enzyme to switch it on. Lab research suggests 1,3-DMBA may weakly block CYP2D6, one of the liver enzymes that helps clear lidocaine. In theory, that could let a little more lidocaine linger in your body and slightly bump up its effects.

I want to be honest with you: this is only test-tube data, the effect is rated minor, and most lidocaine you'd use (a patch, cream, or gel on the skin) barely enters your bloodstream, so a real problem is unlikely. My bigger worry is 1,3-DMBA itself, which is an unapproved stimulant linked to serious heart and stroke risks. Please skip it and check with your pharmacist or doctor.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
