1,3-dmba and Fluphenazine

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Why this applies to Fluphenazine

Fluphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmba interacts with.

Theoretically, 1,3-DMBA might modestly increase the levels of CYP2D6 substrates.
In vitro research shows that 1,3-DMBA weakly inhibits CYP2D6 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body uses an enzyme called CYP2D6 to help clear fluphenazine. Lab studies suggest 1,3-DMBA may weakly block that same enzyme. If it does, fluphenazine could clear a little slower and build up slightly, which might nudge up its usual effects, things like drowsiness, dizziness, a drop in blood pressure when you stand, or restlessness and muscle stiffness.

I want to be honest about how shaky this is. The evidence comes only from test-tube work, the effect is rated minor, and it's described as weak. My bigger worry is 1,3-DMBA itself. It's a banned, unapproved stimulant that can raise heart rate and blood pressure, and that's a rough mix with any antipsychotic. Please skip it and check with your pharmacist or doctor.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

From our pharmacist’s overview on the Fluphenazine page

“Take real care with supplements while on fluphenazine (Prolixin), an antipsychotic used mainly for schizophrenia. Five of the 148 ingredients on file rate major: herbs like henbane that stack anticholinergic side effects (dry mouth, blurred vision, confusion) onto a drug already prone to them, two rauwolfia plants that can intensify it…”
