1,3-dmba and Formoterol

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Why this applies to Formoterol

Formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmba interacts with.

Theoretically, 1,3-DMBA might modestly increase the levels of CYP2D6 substrates.
In vitro research shows that 1,3-DMBA weakly inhibits CYP2D6 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your Formoterol inhaler. In lab dishes, 1,3-DMBA appears to weakly block an enzyme called CYP2D6, which is one of the pathways your body uses to clear Formoterol. If that enzyme slows down, a little more Formoterol could linger in your system. The theory is that you might feel more of its side effects, things like a fast or pounding heartbeat, jitteriness, shaky hands, or a headache.

Honestly though, this is a weak, test-tube-only finding, so it's rated minor. My bigger worry is the 1,3-DMBA itself. It's a banned stimulant that raises heart rate and blood pressure, and stacking that on top of Formoterol's own heart-revving effect is the real concern. I'd avoid this supplement and check in with your pharmacist or doctor.

From our pharmacist’s overview on the Formoterol page

“Formoterol (Foradil, Perforomist) is the long-acting half of many asthma and COPD inhalers, keeping airways open for hours, and its supplement list is calmer than its length suggests. None of the 103 ingredients on file is rated major.”
