The combination pill you're asking about has three ingredients, and the one that matters most here is the caffeine. Caffeine is itself a stimulant, and 1,3-DMBA is a powerful synthetic stimulant too. Stacking two stimulants can add up, so the worry is mostly about your heart and blood vessels: a faster or pounding heartbeat, higher blood pressure, jitteriness, and trouble sleeping. The codeine and acetaminophen parts aren't the concern in this particular interaction.

To be fair, this is a theoretical concern based mainly on animal data, so we can't say exactly how strong it is in people. That said, 1,3-DMBA is a product I'd steer clear of entirely. The FDA considers it adulterated, and it carries real heart and stroke risks. Please check with your pharmacist or doctor before mixing the two.

Theoretically, 1,3-DMBA might modestly increase the levels of CYP2D6 substrates.
In vitro research shows that 1,3-DMBA weakly inhibits CYP2D6 enzymes.

Here's the wrinkle with this combo product: the codeine part is actually a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is what gives you the pain relief. The lab research suggests 1,3-DMBA only weakly blocks that enzyme, so in theory it could slightly slow that conversion and give you a touch less pain relief from the codeine. This is rated minor, and the evidence is just test-tube data, so it's far from a sure thing.

Honestly, my bigger worry isn't the enzyme. 1,3-DMBA is a risky, FDA-flagged stimulant that can spike your heart rate and blood pressure, and the caffeine here adds to that. Please skip products with 1,3-DMBA, and check with your pharmacist or doctor.
