Here's the piece that matters most: Fiorinal with Codeine already contains a stimulant, caffeine, right in the mix. Stacking that with 1,3-DMBA, which acts like the banned stimulant DMAA, means you'd essentially be doubling up on stimulant effects. On paper, that can push your heart rate and blood pressure higher, and may bring on jitteriness, a pounding or racing heartbeat, anxiety, or trouble sleeping.

To be straight with you, the evidence here is mostly animal and lab data, so the heart strain is a theoretical concern rather than something proven in people. Even so, I'd steer clear: 1,3-DMBA is an unapproved, unsafe ingredient the FDA flags as adulterated, with real heart and stroke risks. Please talk with your pharmacist or doctor before mixing anything with it.

Theoretically, 1,3-DMBA might modestly increase the levels of CYP2D6 substrates.
In vitro research shows that 1,3-DMBA weakly inhibits CYP2D6 enzymes.

Here's the wrinkle with Fiorinal with Codeine: the codeine part is actually a prodrug. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what gives the real pain relief. The lab research on file suggests 1,3-DMBA may weakly block that same enzyme. If it does, less codeine gets turned into morphine, so you might get weaker pain relief from this medication.

Honestly, this is only theoretical, based on test-tube data and rated as minor, so I wouldn't lose sleep over the codeine angle. My bigger worry is the stimulant itself. 1,3-DMBA is an unsafe, FDA-flagged ingredient that raises heart rate and blood pressure, and stacking it with the caffeine already in this pill is a real strain on your heart. I'd skip 1,3-DMBA and check with your pharmacist.
