The thing to watch here is the caffeine in this combination product. Caffeine is itself a stimulant, and 1,3-DMBA is a powerful stimulant too. When you stack two stimulants together, their effects can add up. That may mean a racing or pounding heart, higher blood pressure, jitteriness, and trouble sleeping. In some people that extra strain on the heart and blood vessels is what raises real concern.

I'll be honest: this warning comes mostly from animal research, so the human evidence is thin. But 1,3-DMBA is a banned, unapproved ingredient that the FDA considers adulterated, and it has been linked to serious heart and stroke risks on its own. The aspirin and codeine parts aren't really the issue here. My honest advice is to avoid 1,3-DMBA altogether, and talk with your pharmacist or doctor before mixing anything.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Theoretically, 1,3-DMBA might modestly increase the levels of CYP2D6 substrates.
In vitro research shows that 1,3-DMBA weakly inhibits CYP2D6 enzymes.

Here's the wrinkle with this combo: the codeine part of your medicine is a prodrug. Your body has to switch it on using an enzyme called CYP2D6, and that's what turns codeine into its active pain-relieving form. In lab dish studies, 1,3-DMBA seems to weakly block that same enzyme. If it slows the switch, you could actually get less pain relief from the codeine, not more.

Honestly though, this is theory based only on test-tube data, and the effect is rated minor and weak, so it may not matter much in real life. The bigger concern is 1,3-DMBA itself, a risky stimulant the FDA considers unsafe. I'd skip products containing it and check with your pharmacist or doctor first.
