Why this applies to Dexamethasone

Dexamethasone falls under CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might inhibit CYP2B6.
An in vitro study shows that silybin, a constituent of milk thistle, binds to and noncompetitively inhibits CYP2B6. Additionally, silybin might downregulate the expression of CYP2B6 by decreasing mRNA and protein levels.

Here's the gist for combining milk thistle with dexamethasone. Lab and animal studies suggest a compound in milk thistle called silybin may block an enzyme called CYP2B6, which is one of the routes your body uses to break down dexamethasone. If that enzyme slows down, slightly more dexamethasone could linger in your system, which in theory could nudge up its usual effects, like trouble sleeping, mood changes, higher blood sugar, fluid retention, or stomach upset.

I'll be honest though: this is only seen in test-tube and animal research, not in real people, so the effect may be small or not show up at all. Dexamethasone is an active drug, not a prodrug, so the direction makes sense. Before adding milk thistle, just run it by your pharmacist or doctor.

P-GLYCOPROTEIN SUBSTRATES

Dexamethasone also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist for these two together. Dexamethasone is carried around your body in part by a transporter called P-glycoprotein, and milk thistle might slow that transporter down a little. In theory, that could let a bit more dexamethasone get absorbed, which would mean slightly stronger steroid effects, things like fluid retention, higher blood sugar, mood changes, or trouble sleeping.

The good news is this is a minor, mostly theoretical concern. The lab signal didn't really pan out in people. In a human study, milk thistle didn't change levels of digoxin, another drug that relies on this same transporter. So for most folks taking dexamethasone, milk thistle is unlikely to cause a real problem. Still, it's worth a quick mention to your pharmacist or doctor, especially if you take dexamethasone regularly.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version: your body uses an enzyme called CYP3A4 to clear dexamethasone. There's a theory that milk thistle might slow that enzyme down a bit, which could let dexamethasone linger a little longer and, in theory, raise the chance of typical steroid side effects like trouble sleeping, mood changes, higher blood sugar, fluid retention, or stomach upset.

That said, the evidence here is thin and conflicting. Human studies on milk thistle and CYP3A4 drugs mostly showed no meaningful change, so for most people taking dexamethasone this is unlikely to cause a real problem. Still, it's worth a quick mention to your pharmacist or doctor, especially on long courses or higher doses, so they can keep an eye on things.

About Dexamethasone

What Is Dexamethasone Used For?

Brand names: Decadron, Hexadrol, Dexone, Ozurdex, Dexycu, Hemady, Dextenza

Dexamethasone is used to treat a wide range of conditions depending on the form and route. Oral tablets and solutions treat severe allergic reactions (such as asthma, contact dermatitis, and seasonal allergic rhinitis), inflammatory skin diseases, endocrine disorders including adrenal insufficiency and congenital adrenal hyperplasia, gastrointestinal diseases such as ulcerative colitis and regional enteritis, blood disorders, rheumatic conditions, kidney disease, and respiratory conditions including tuberculosis complications. Hemady (oral tablet) is specifically approved, in combination with other medicines, for multiple myeloma in adults. TaperDex (6-day and 12-day oral tablet packs) are used for the same broad range of allergic, inflammatory, and endocrine conditions.

Injectable forms (intramuscular and intravenous) are used when oral treatment is not possible, covering endocrine emergencies, shock, rheumatic disorders, and more. Eye-specific products treat different conditions: Dextenza (intracanalicular insert) treats eye inflammation and pain after eye surgery and itching from allergic conjunctivitis. Ozurdex (intravitreal implant) treats macular edema from retinal vein occlusion and diabetic macular edema, and non-infectious uveitis in the back of the eye. Dexycu (intraocular injection) treats inflammation after eye surgery. Maxidex (ophthalmic suspension) and dexamethasone sodium phosphate eye drops treat steroid-responsive inflammatory conditions of the eye. The ophthalmic/otic solution also treats steroid-responsive inflammation of the outer ear canal. A topical ointment (Seacall) is used for skin conditions including contact dermatitis, eczema, and neurodermatitis.
