Why this applies to Erdafitinib

Erdafitinib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist for erdafitinib (Balversa). Your body partly clears this cancer drug using a liver enzyme called CYP2C9. Milk thistle might slow that enzyme down a little. If it does, erdafitinib could clear more slowly and build up, which in theory could mean stronger side effects like dry mouth and eyes, mouth sores, changes in your fingernails or skin, diarrhea, or vision and eye problems.

The honest part: the evidence here is shaky and conflicting. Some lab studies hint at it, a few case reports raised flags with other cancer drugs, but solid human studies didn't show much effect. It's rated minor and unlikely. Still, since erdafitinib is serious medicine, please check with your oncology pharmacist or doctor before adding milk thistle.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the situation with milk thistle and erdafitinib (Balversa). Your body uses an enzyme called CYP3A4 to help clear erdafitinib, and there's a theory that milk thistle's silymarin might slow that enzyme down. If that happened, more erdafitinib could linger in your system, which might mean stronger side effects like mouth sores, dry eyes or vision changes, diarrhea, dry skin, or high phosphate levels.

That said, the actual evidence here is thin. Human studies showed milk thistle did not meaningfully change how the body handles similar CYP3A4 drugs, so this concern is mostly theoretical. Because erdafitinib is a serious cancer medicine, I'd still loop in your oncology team or pharmacist before adding milk thistle, rather than guessing.

About Erdafitinib

What Is Erdafitinib Used For?

Brand name: Balversa

Balversa (erdafitinib) is used to treat adults with locally advanced or metastatic urothelial carcinoma (UC) — cancer of the bladder or urinary tract — that has spread and has not responded to, or has gotten worse after, at least one prior round of systemic treatment. Patients must have a specific type of FGFR3 genetic alteration in their tumor, confirmed by an FDA-approved test before starting.

Balversa is not recommended for patients who are eligible for — but have not yet received — a PD-1 or PD-L1 inhibitor (a type of immunotherapy). Those therapies should be tried first when appropriate.
