Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the gist for your cough medicine. Your body relies on a liver enzyme called CYP2D6 to break down dextromethorphan and clear it out. Schizonepeta may slow that enzyme down, which means the dextromethorphan could hang around longer and build up a bit more than usual.

If that happens, you might notice more of its typical effects, like drowsiness, dizziness, lightheadedness, or feeling a little foggy or off-balance. The good news is this concern comes only from lab and animal studies, not from people, so it's theoretical and far from certain. Still, if you take both, keep an eye out for extra grogginess, and check with your pharmacist or doctor before combining them.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the basic idea with Schizonepeta and your cough medicine. In animal studies, a compound in this herb seems to speed up a liver enzyme called CYP3A4, which is one of the pathways that helps clear dextromethorphan from your body. If that enzyme works faster, your body could break down the cough suppressant a bit quicker, which might make it less effective at quieting your cough.

I want to be honest with you though: this is only a theoretical concern based on lab and animal data, not real human studies, so it may not amount to much in practice. Dextromethorphan is also pretty safe and short-term. If you notice your cough just isn't settling, mention it to your pharmacist or doctor before changing anything.

What Is Dextromethorphan Used For?

Brand names: Hold, Benylin Adult, Benylin Pediatric, Theraflu Thin Strips Long Acting Cough, Neo Citran Thin Strips Cough

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
