Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the short version: your body relies heavily on a liver enzyme called CYP2D6 to clear Rexulti (brexpiprazole) out of your system. Schizonepeta has shown, mostly in lab and animal tests so far, that one of its compounds may slow that same enzyme down. If that happens in people, your body would break down brexpiprazole more slowly, so the drug could build up a bit higher than expected.

What might that feel like? Possibly more of brexpiprazole's usual effects, like drowsiness, restlessness, dizziness, or feeling unsteady. I want to stress this is theoretical and based on weak evidence, not human studies, so it may not amount to much. Still, since brexpiprazole affects your brain, it's worth a quick chat with your pharmacist or doctor before combining the two.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Rexulti (brexpiprazole): your liver uses an enzyme called CYP3A4 to help clear it out. Schizonepeta contains a compound that, in animal studies, seems to speed up that enzyme. If that holds true in people, your body might break down brexpiprazole a little faster, which could leave you with less medicine working and a higher chance that your symptoms aren't fully controlled.

I want to be honest about the strength here: this is based only on lab and animal research, not real patient studies, so it's more of a theoretical concern than a proven problem. Still, since brexpiprazole keeps your mood and thinking steady, it's worth a quick chat with your pharmacist or doctor before adding this herb. Don't change your dose on your own.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
