Why this applies to Fentanyl

Fentanyl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the thing about this one: the worry on file is that Schizonepeta might block an enzyme called CYP2D6, which could let drugs handled by that enzyme build up. But fentanyl is actually a bit of an odd fit here. Fentanyl is broken down mainly by a different enzyme, CYP3A4, not CYP2D6. So in real life, this particular interaction is unlikely to have much effect on your fentanyl levels.

That said, fentanyl is a powerful opioid where even small changes matter, and the evidence here is only from lab and animal studies, so it's theoretical at best. If you'd feel any extra sleepiness, slowed breathing, or confusion, take it seriously. Please run any new supplement by your pharmacist or doctor before combining it with fentanyl.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the basic idea with these two. Your body relies on a liver enzyme called CYP3A4 to break down fentanyl. Animal research hints that a compound in Schizonepeta may speed that enzyme up. If that holds true in people, your body could clear fentanyl a little faster than usual, which might mean less pain relief than you're counting on.

I want to be honest about the strength of this, though. It's based only on lab and animal data, so it's theoretical and not proven in humans. Still, fentanyl is a powerful opioid where steady control matters, so I wouldn't change anything on your own. If your breakthrough pain seems to creep back, or you're thinking about adding this herb, please loop in your pharmacist or doctor first.

What Is Fentanyl Used For?

Brand names: Sublimaze, Duragesic, Fentora, Lazanda, Abstral, Subsys

Fentanyl transdermal system is prescribed for severe, persistent pain that cannot be controlled with other pain medicines, including shorter-acting opioids. It is only for people who are already opioid-tolerant — meaning they've been taking a significant daily opioid dose for at least a week.

It is not for short-term pain, post-surgical pain, mild pain, or use on an as-needed basis. Because of serious risks including addiction, overdose, and death, it is reserved for situations where other treatment options have failed or are not appropriate.
