Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Your cough and cold combo pairs an antihistamine (chlorpheniramine) with a cough suppressant (dextromethorphan). Both lean on a liver enzyme called CYP2D6 to be cleared out. Schizonepeta, in early lab and animal work only, seems to slow that same enzyme down. In theory, that could let these two ingredients linger a bit longer and feel stronger.

What might that look like? More drowsiness, dizziness, dry mouth, or in higher amounts, a foggy or spacey feeling from the dextromethorphan. To be clear, this is theoretical and based on animal data, not proven in people, so it may not amount to much. Still, if you plan to use both, it's worth a quick chat with your pharmacist, especially before driving.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

This combo cold medicine has two active ingredients. Dextromethorphan, the cough suppressant, is partly processed by an enzyme called CYP3A4 (and also CYP2D6). The concern on file is that a compound in Schizonepeta may speed up CYP3A4. In theory, that could clear dextromethorphan a bit faster, so your cough relief might feel weaker than expected. The chlorpheniramine (antihistamine) piece isn't really the focus here.

Honestly, this is a very weak, theory-only signal based on animal and lab work, not real people, so I wouldn't lose sleep over it. Neither ingredient is a prodrug, so we're not flipping anything. If you take Schizonepeta regularly and your cough syrup seems less effective, just check with your pharmacist or doctor.

Some substrates of CYP2E1 include acetaminophen, chlorzoxazone (Parafon Forte), ethanol, theophylline, and anesthetics such as enflurane (Ethrane), halothane (Fluothane), isoflurane (Forane), and methoxyflurane (Penthrane).

Here's the honest answer: the interaction on file is about a liver enzyme called CYP2E1, and Schizonepeta may slow it down in lab and animal studies. But neither chlorpheniramine nor dextromethorphan really leans on CYP2E1 to be cleared. Chlorpheniramine is handled mostly by CYP2D6 and others, and dextromethorphan goes mainly through CYP2D6 and CYP3A4. So this particular pathway isn't a strong match for your cough and cold medicine.

Neither ingredient is a prodrug here, so I wouldn't expect a meaningful change in their effects. This is also very weak, theoretical evidence. Still, both can already cause drowsiness, dizziness, and a dry mouth, so go easy and check with your pharmacist before combining them.
