Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for bedaquiline (Sirturo): your body relies on a liver enzyme called CYP3A4 to clear this drug. Animal studies hint that a compound in Schizonepeta may speed that enzyme up. If that holds true in people, your body could clear bedaquiline a little faster, leaving less of it to fight your TB infection. With a serious condition like multi-drug resistant TB, you really don't want anything quietly weakening your treatment.

I want to be honest that this is theoretical and comes only from lab and animal work, so we can't say for sure it happens in humans. Still, because bedaquiline matters so much, I'd ask your doctor or pharmacist before adding Schizonepeta rather than risk it.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
