Estradiol, Norethindrone Acetate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 1A2. Theoretically, schizonepeta might increase the effects and side effects of CYP1A2 substrates.
Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

Activella combines two hormones, estradiol (an estrogen) and norethindrone (a progestin), to ease menopause symptoms. The concern here is that Schizonepeta may slow down a liver enzyme called CYP1A2, which plays a part in clearing estrogen. If estradiol is cleared a bit more slowly, its levels could nudge upward, which in theory might bring more estrogen-type effects or side effects like breast tenderness, bloating, nausea, headaches, or spotting.

Here is the honest part: this is based only on animal and lab research, not human studies, so it is very much theoretical and not something I'd lose sleep over. Still, it never hurts to mention any new herbal product to your pharmacist or doctor before starting it.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Your Activella combines estradiol with norethindrone, and both hormones are partly broken down by a liver enzyme called CYP3A4. The concern here is that schizonepeta may speed up that enzyme. If it does, your body could clear these hormones a little faster, which might soften their effect. In practice that could mean breakthrough bleeding, spotting, or the return of menopausal symptoms like hot flashes and night sweats.

I want to be honest with you though: this is based only on animal and lab research, not on real people, so it is quite theoretical and may never amount to anything noticeable. Still, since it is worth a quick check, mention the herb to your pharmacist or doctor before combining the two. Please don't stop or change your Activella on your own.
