Toremifene Citrate falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the gist with milk thistle and your toremifene (Fareston). Toremifene is an active medication on its own, not a prodrug, and part of how your body clears it involves a phase 2 process called glucuronidation. In lab and animal studies, compounds in milk thistle (the silymarin group) can slow down the UGT enzymes that handle this step. The thinking is that if clearance slows, toremifene levels could creep up a bit.

If that happened, you might notice more of the usual toremifene effects, like hot flashes, sweating, nausea, or dizziness. I want to be honest though: this is theoretical, based only on test-tube and animal data, so we don't know if it matters in real life. To be safe, just check with your oncologist or pharmacist before adding milk thistle.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: your body uses a liver enzyme called CYP3A4 to break down toremifene (Fareston). The thinking is that milk thistle might slow that enzyme a little, which could let toremifene build up slightly and, in theory, raise the odds of side effects like hot flashes, nausea, dizziness, or sweating. Toremifene isn't a prodrug, so it doesn't need that enzyme to work, it just needs it for clearance.

Honestly, the evidence here is thin. Lab results are mixed, and careful human studies haven't shown milk thistle meaningfully changing how similar drugs are cleared. This is rated minor and unlikely. Still, since this is a cancer medicine, I'd run it past your oncologist or pharmacist before adding milk thistle.
