Why this applies to Apixaban

Apixaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: your body partly clears Eliquis (apixaban) using a liver enzyme called CYP3A4. The theory is that milk thistle's silymarin might slow that enzyme down a bit, which could let a little more apixaban hang around. Since apixaban is a blood thinner, more of it in your system could mean a higher chance of bruising or bleeding, like nosebleeds, bleeding gums, or blood in your urine or stool.

The good news is that this concern is mostly theoretical. Human studies have shown milk thistle doesn't meaningfully change how the body handles other CYP3A4 drugs, so a real-world problem is unlikely. Still, because apixaban is a blood thinner where balance matters, it's worth a quick word with your pharmacist or doctor before adding milk thistle, and watch for any unusual bleeding.

P-GLYCOPROTEIN SUBSTRATES

Apixaban also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist for Eliquis (apixaban): your body uses a kind of cellular pump called P-glycoprotein to move this blood thinner around. Milk thistle, in lab dishes, can slow that pump down a little. In theory, that could let a touch more apixaban get absorbed, which might nudge up your bleeding risk (think easier bruising, nosebleeds, or bleeding that's slow to stop).

But I want to keep this in perspective. The real-world evidence for this is weak and mostly theoretical. A similar study with another pump-dependent drug showed milk thistle didn't actually change its levels. So for most people this is unlikely to cause trouble. Still, since apixaban is a blood thinner, it's smart to run it past your pharmacist or doctor before adding milk thistle, and watch for any unusual bleeding.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
