Acetaminophen, Butalbital, Caffeine, Codeine falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the gist of it. This combination pill (Fioricet with Codeine) clears partly through a liver process called glucuronidation. In lab dishes and animal studies, milk thistle's silymarin slowed down the UGT enzymes that handle that step. In theory, that could let some of these ingredients hang around a little longer than usual. With the acetaminophen and butalbital pieces especially, more buildup could mean extra drowsiness, dizziness, or sleepiness.

I'll be straight with you though: this is only a theoretical concern from test-tube work, not something proven in people, so I wouldn't lose sleep over it. Still, since this medicine already causes sedation, it's worth a quick chat with your pharmacist or doctor before adding milk thistle.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

This combination pill has a few moving parts, and milk thistle's connection to it is pretty weak. The main question is whether milk thistle slows down a liver enzyme called CYP3A4. The honest answer is that the research is all over the map. Careful human studies showed milk thistle at typical doses did not meaningfully change how the body handles CYP3A4 medicines, so any effect on the butalbital or codeine portion here is unlikely to matter for most people.

If milk thistle did slow things down, you might in theory notice a bit more drowsiness, dizziness, or constipation. That's only a theoretical concern though, based mostly on lab work and a handful of case reports with strong cancer drugs. It's still worth a quick word with your pharmacist or doctor before adding it.
