Irinotecan Hydrochloride falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the situation with milk thistle and your irinotecan (Onivyde). Your body relies on a phase 2 enzyme called UGT (glucuronidation) to deactivate and clear out irinotecan's active form, known as SN-38. In lab and animal studies, the silymarin compounds in milk thistle appear to slow that enzyme down. In theory, if the cleanup process is blunted, more of that active chemo byproduct could linger, which could mean rougher side effects like severe diarrhea, low white blood cell counts, nausea, and fatigue.

I want to be honest about the strength of this: it is theoretical, based only on test-tube and animal data, not real patient outcomes. Still, because chemo has such a narrow safety margin, I would not combine them on your own. Please check with your oncology team or pharmacist first.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the good news on this one. The theory was that milk thistle (specifically its silymarin compounds) might slow down CYP3A4, an enzyme that helps process irinotecan. If that happened, more drug could linger in your body and push side effects like severe diarrhea, low blood counts, nausea, and fatigue. But actual human studies that paired milk thistle (420 to 1350 mg a day) with irinotecan found no meaningful change in how the drug was handled.

So this is rated minor and unlikely. A handful of older case reports did mention extra toxicity with milk thistle and other cancer drugs, so I never wave it off completely. Since irinotecan is serious chemo, please run any supplement past your oncology team before adding it.

Irinotecan Hydrochloride also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the gist for irinotecan (Onivyde). Milk thistle's silymarin can block a transporter protein called OATP-B1, which helps move certain drugs into your liver and tissues. Irinotecan moves through these transporter systems, so in theory milk thistle could shift how much of it gets where it needs to go.

Honestly, the evidence here is thin. It comes mostly from lab dish studies, plus a couple of scattered reports involving other cancer drugs, not irinotecan itself. So I wouldn't lose sleep over it, but I also wouldn't add a concentrated milk thistle supplement during chemo without a green light. Irinotecan already carries real risks like severe diarrhea and low blood counts, so please run this by your oncology team before combining the two.
