Sotagliflozin falls under ANTIDIABETES DRUGS — the group Milk Thistle interacts with.

Taking milk thistle with antidiabetes drugs may increase the risk of hypoglycemia.
Clinical research shows that milk thistle extract, alone or along with tree turmeric extract, can lower blood glucose levels and glycated hemoglobin (HbA1c) in patients with type 2 diabetes, including those already taking antidiabetes drugs. Additionally, animal research shows that milk thistle extract increases the metformin maximum plasma concentration and area under the curve and decreases the renal clearance of metformin, due to inhibition of the multi-drug and toxin extrusion protein 1 (MATE1) renal tubular transport protein.

Here's the short version for anyone taking sotagliflozin (Inpefa) alongside milk thistle. Milk thistle itself may nudge your blood sugar a little lower, and some research shows it can lower both glucose and HbA1c in people with type 2 diabetes, even those already on diabetes medicines. Stack that gentle sugar-lowering effect on top of a drug that's already working to control your glucose, and you could end up a bit lower than expected.

With sotagliflozin, watch for the usual signs of a low: shakiness, sweating, dizziness, or feeling foggy. This is based on lower-quality studies, so it isn't a guarantee, and the effect is likely modest. Please don't start, stop, or change anything on your own. Just loop in your pharmacist or doctor so they can keep an eye on your numbers.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the short version: sotagliflozin (Inpefa) is partly cleared from your body through a process called glucuronidation, and lab studies hint that milk thistle's silymarin can slow the enzymes (UGT) that do that work. In theory, that could let a little more sotagliflozin build up. If levels ran higher, you might notice its usual effects more strongly, things like extra urination, dehydration, dizziness, or low blood sugar if you also take insulin or a sulfonylurea.

The important part: this is only test-tube and animal research, not something proven in people, so I wouldn't lose sleep over it. Still, if you'd like to add milk thistle, run it by your pharmacist or doctor first and keep an eye on how you feel.
