Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

This combination cold product includes acetaminophen (Tylenol), which your liver partly clears through a process called glucuronidation. In test-tube studies, milk thistle compounds can slow down the enzyme (UGT) that runs this process. In theory, that could let a bit more acetaminophen hang around, which matters because high acetaminophen levels are hard on the liver.

Here's my honest take: this is only lab and animal data, so we don't have real proof it happens in people taking normal doses. None of these ingredients are prodrugs, so we're not worried about them losing effect. The bigger, practical reminder is to never double up on acetaminophen from multiple products and to stay within the daily limit. If you have any liver concerns or take milk thistle regularly, just run it by your pharmacist first.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

This combo cold-and-flu product packs four ingredients: acetaminophen (pain and fever), chlorpheniramine (antihistamine), dextromethorphan (cough), and pseudoephedrine (decongestant). The theory here is that milk thistle might slow down an enzyme called CYP3A4, which helps break down dextromethorphan. If that happened, this one ingredient could build up a bit and lead to more drowsiness or dizziness.

Honestly, the evidence for this is weak and conflicting. Human studies show milk thistle usually does not meaningfully change how CYP3A4 medicines are handled, and any effect on a short-term cold product would likely be small. I wouldn't lose sleep over it, but if you take milk thistle daily, just mention it to your pharmacist or doctor so they can give you the all-clear.
