Why this applies to Amitriptyline

Amitriptyline falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist for amitriptyline and milk thistle. One pathway your body uses to break down amitriptyline is a liver enzyme called CYP2C9. In a test tube, milk thistle (silymarin) might slow that enzyme down a little. If that happened in real life, your body could clear amitriptyline a bit slower, leaving slightly more of it around. With this medicine that could mean more of its usual side effects, like drowsiness, dry mouth, constipation, dizziness, or a faster heartbeat.

The good news is the evidence here is weak and conflicting. Real human studies haven't shown milk thistle meaningfully blocking this enzyme, so a real-world problem is unlikely and minor. Still, if you plan to combine them, give your pharmacist or doctor a heads-up so they can keep an eye on how you feel.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for amitriptyline (Elavil). The body partly uses an enzyme called CYP3A4 to help clear this medicine. The theory is that milk thistle might slow that enzyme down, which could let a little more amitriptyline linger in your system. If that happened, you might notice more of its usual side effects, like drowsiness, dry mouth, constipation, blurred vision, or feeling lightheaded when you stand up.

That said, the evidence here is weak and mostly from lab dishes, and human studies suggest milk thistle doesn't meaningfully block this enzyme. So a real problem is unlikely. Still, it's worth a quick mention to your pharmacist or doctor, especially if you start feeling unusually groggy or off.
