Why this applies to Macitentan, Tadalafil

Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Opsynvi combines two blood pressure lowering medicines used for pulmonary arterial hypertension, and part of how your body clears them runs through a liver enzyme called CYP2C9. The thinking here is that milk thistle might slow that enzyme a little. If it did, these medicines could hang around longer and their effects could stack up, meaning things like headache, flushing, low blood pressure, dizziness, or an upset stomach might feel stronger.

Here is the honest part: the evidence is shaky and conflicting. Some lab studies hint at this, but better human research suggests milk thistle does not meaningfully block CYP2C9. So a real problem is unlikely. Still, if you notice more lightheadedness or headaches after adding milk thistle, mention it to your pharmacist or doctor before making any changes.

Macitentan, Tadalafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Opsynvi combines two medicines, macitentan and tadalafil, and both are partly cleared by a liver enzyme called CYP3A4. The idea here is that milk thistle might slow that enzyme down, which in theory could let these drugs build up a bit. If that happened, you could see more of their usual effects, like low blood pressure, dizziness, headache, flushing, or a stuffy nose.

Honestly though, the evidence for this is weak. Lab results conflict, and good human studies show milk thistle doesn't meaningfully change how CYP3A4 handles similar drugs. So for most people this is unlikely to cause a real problem. Still, since Opsynvi affects blood pressure, it's worth mentioning to your pharmacist or doctor before adding milk thistle, just so someone who knows your full picture can weigh in.
