Sulfamethoxazole, Trimethoprim falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the short version for Bactrim (sulfamethoxazole/trimethoprim) and milk thistle: this is a very minor, mostly theoretical concern. The sulfamethoxazole part is partly handled by a liver enzyme called CYP2C9, and milk thistle might slow that enzyme down a bit. In theory, that could let a little more of the drug build up. If that happened, you might notice slightly more of the usual Bactrim side effects, like nausea, stomach upset, or a skin rash.

The honest truth is the research here is conflicting, and good human studies suggest milk thistle does not meaningfully block this enzyme. For a short antibiotic course, I wouldn't lose sleep over it. Still, mention both to your pharmacist so they can keep an eye on things, especially if you notice any rash.
