Here's the situation with Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) and milk thistle. Sofosbuvir is actually a prodrug, meaning your body has to convert it into its active form to fight the hepatitis C virus. There's a concern, based only on animal (rat) studies, that milk thistle could interfere with that activation and reduce how much of the active drug reaches your liver. In theory, that could make your treatment less effective at clearing the virus, which matters a lot when you're trying to cure hepatitis C.

I want to be honest that this is theoretical and comes from lab and animal data, not human studies, so we don't know if it truly happens in people. Still, because finishing a full, effective course is so important, I'd skip milk thistle during treatment unless your doctor or pharmacist okays it.

An in vitro study shows that silybin, a constituent of milk thistle, binds to and noncompetitively inhibits CYP2B6. Additionally, silybin might downregulate the expression of CYP2B6 by decreasing mRNA and protein levels.

Here's the honest picture with milk thistle and Vosevi (sofosbuvir, velpatasvir, voxilaprevir). The concern on file is that a milk thistle compound called silybin might block a liver enzyme known as CYP2B6. In theory, slowing that enzyme could let a little more of one of these drugs build up. But this is based only on test-tube work, not on real people, so it's a weak, mostly hypothetical flag.

If anything did shift, you might notice a bit more of the usual side effects like headache, tiredness, or nausea. There's also a bigger-picture point: since you're treating hepatitis C, I'd rather your liver be stable, and adding herbal products without a clear benefit isn't worth the unknowns. Please run it by your pharmacist or doctor before combining them.

P-GLYCOPROTEIN SUBSTRATES

Sofosbuvir, Velpatasvir, Voxilaprevir also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the short version: all three medicines in Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) are carried around the body by a transporter called P-glycoprotein. In the lab, milk thistle's silymarin can slow that transporter down a bit, which could in theory let a little more of the medicine get absorbed. If that happened, you might notice mild things like headache, nausea, or tiredness.

The good news is the real-world evidence is reassuring. When milk thistle was tested with another P-glycoprotein drug (digoxin) in healthy people, it didn't meaningfully change absorption. So this interaction is considered minor and unlikely. Still, since Vosevi treats your hepatitis C and milk thistle is marketed for the liver, it's worth a quick chat with your pharmacist or doctor before combining them.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist with Vosevi (sofosbuvir, velpatasvir, voxilaprevir) and milk thistle. Part of this combo is handled by a liver enzyme called CYP3A4, and in theory milk thistle's silymarin might slow that enzyme down. If it did, the medicine could build up a bit and you might notice more of its usual side effects, like headache, tiredness, or nausea. The honest truth, though, is that the evidence here is weak and conflicting. Careful human studies showed milk thistle did not meaningfully change how similar CYP3A4 drugs were processed.

So for most people this is a minor, unlikely concern. Since you'd be using both for your liver, I'd still run it past your pharmacist or doctor before adding milk thistle, just to be safe.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults who do not have cirrhosis or who have compensated cirrhosis (mild liver scarring that is still functioning). It is specifically for people who have already been treated with an HCV regimen before and need a different option.

Vosevi is approved for adults with HCV genotype 1, 2, 3, 4, 5, or 6 who previously took a medicine that included an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who previously took sofosbuvir without an NS5A inhibitor.
