Belumosudil Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist with Rezurock (belumosudil) and milk thistle. Belumosudil is handled by a liver enzyme called CYP3A4. Milk thistle's active part, silymarin, might slow that enzyme down a little, but the evidence is genuinely mixed. In careful human studies, milk thistle didn't meaningfully change how the body cleared several similar CYP3A4 medicines.

If it did slow things down, belumosudil could build up a bit, which in theory might mean more of its usual side effects like nausea, tiredness, headache, or changes in liver blood tests. Honestly though, this is a minor, unlikely concern based mostly on lab work, not strong patient data. Belumosudil is not a prodrug, so it doesn't flip. Still, since you're managing GVHD, please run any milk thistle use past your transplant team or pharmacist first.

Rezurock (belumosudil) is used to treat chronic graft-versus-host disease (chronic GVHD) — a serious immune condition that can occur after a stem cell or bone marrow transplant, where the donor's immune cells attack the recipient's body. It is specifically for patients aged 12 and older whose chronic GVHD has not responded to at least two prior systemic (whole-body) treatments.
