Niacin, Simvastatin falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the gist with Simcor (which combines niacin and simvastatin) and milk thistle. In the test tube, milk thistle's silymarin compounds can slow down an enzyme system called UGT, which helps your body package and clear certain drugs. Both niacin and simvastatin lean on glucuronidation as part of how they're handled, so in theory milk thistle could let their levels creep up a bit.

If that happened, you might notice more of the usual effects: niacin's flushing, warmth, or itching, and a higher chance of statin-related muscle aches. I want to be honest, though, this is only lab and animal data, not something proven in people. It's worth a quick mention to your pharmacist or doctor, but no need to panic.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the honest picture with Simcor, which combines simvastatin and niacin. The simvastatin part is cleared by a liver enzyme called CYP3A4. In theory, if milk thistle slowed that enzyme down, simvastatin could build up and raise the odds of side effects like muscle aches, weakness, or (rarely) more serious muscle breakdown. Niacin can add to that muscle risk too.

The good news: this is mostly a lab-based worry. Real human studies show milk thistle doesn't meaningfully change CYP3A4 medicines, so an actual problem here is unlikely. Still, since simvastatin is sensitive to CYP3A4, it's worth a quick word with your pharmacist or doctor before adding milk thistle, and let them know if you notice new muscle soreness.

HMG-CoA REDUCTASE INHIBITORS ("Statins")

Niacin, Simvastatin also falls under HMG-CoA REDUCTASE INHIBITORS ("Statins").

Theoretically, milk thistle might interfere with statin therapy by decreasing the activity of organic anion transporting polypeptide 1B1 (OATB1B1) and inhibiting breast cancer resistance protein (BCRP).
Preliminary evidence suggests that a milk thistle extract of silymarin can decrease the activity of the OATP1B1, which transports HMG-CoA reductase inhibitors into the liver to their site of action, and animal research shows this increases the maximum plasma concentration of pitavastatin and pravastatin. The silibinin component also inhibits BCRP, which transports statins from the liver into the bile for excretion. However, in a preliminary study in healthy males, silymarin 140 mg three times daily had no effect on the pharmacokinetics of a single 10 mg dose of rosuvastatin.

Here's the gist with your Simcor (which combines simvastatin and niacin) and milk thistle. The concern is mostly theoretical, coming from lab and animal work, not solid human studies. Milk thistle's silymarin may slow the transporters that move statins like simvastatin into the liver and out into bile. If that happened, a bit more simvastatin could linger in your blood, which in theory could nudge up the risk of statin side effects like muscle aches or tenderness.

That said, this is rated minor and unlikely, and one human study found no real effect. So I wouldn't lose sleep over it. Still, it's worth a quick mention to your pharmacist or doctor, especially if you ever notice unexplained muscle pain or weakness.

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Simcor combines simvastatin with niacin. Here's the part that matters: simvastatin relies on a transporter called OATP1B1 to get pulled into your liver, which is where it does its cholesterol-lowering job. Milk thistle's silymarin can block that transporter, at least in lab dishes. In theory, that could leave a bit less simvastatin reaching the liver, which might soften how well it lowers your LDL.

Honestly, this is weak, test-tube level evidence rather than something proven in real patients, so I wouldn't lose sleep over an occasional cup or capsule. Still, since this is your heart medicine, it's worth a quick mention to your pharmacist or doctor before you add a daily milk thistle supplement, just so they can keep an eye on your numbers.
