Amitriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the gist with these two together. Your body uses an enzyme called CYP2D6 to help break down and clear amitriptyline. Lab studies suggest that yohimbine, the active compound in yohimbe, can slow that same enzyme down. If amitriptyline isn't cleared as quickly, its levels could creep up, which might mean more of its usual side effects, like drowsiness, dry mouth, constipation, dizziness, or a faster heartbeat.

I'll be honest, this is mostly theoretical and comes from test-tube research, not real people, so it may not amount to much for everyone. But yohimbe on its own can raise blood pressure and speed up your heart, and amitriptyline can affect heart rhythm too, so that combo is worth a conversation. Please check with your pharmacist or doctor before pairing them.

TRICYCLIC ANTIDEPRESSANTS (TCAs)

Amitriptyline also falls under TRICYCLIC ANTIDEPRESSANTS (TCAs).

Theoretically, taking yohimbe with TCAs can increase adverse effects.
A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor. The effects of yohimbe bark itself are unclear; oral yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

Here's the gist if you take amitriptyline (Elavil) and are curious about yohimbe. Both can rev up the body's stress chemicals (norepinephrine), so stacking them may push that effect too far. In a small study, people on tricyclics who got yohimbine ran into severe anxiety, agitation, and tremor. With amitriptyline you could also feel a racing or pounding heart, jitteriness, sweating, and a jump in blood pressure.

Now, honesty matters here: this is mostly theoretical, and the study used injected yohimbine, not the bark you'd swallow. We don't really know how much yohimbine your body absorbs from a supplement. Still, since amitriptyline already strains the heart, I'd run any yohimbe use past your pharmacist or doctor first.

Here's the gist: amitriptyline is partly broken down by a liver enzyme called CYP1A2. In lab dish (test tube) studies, yohimbe seemed to speed that enzyme up. If that happened in your body, it could clear amitriptyline a little faster and possibly leave you with less antidepressant effect, so your mood symptoms might not be as well controlled.

I want to be honest about the strength of this, though. This is theoretical, based only on lab research, and rated minor. The bigger real-world worry with yohimbe is its own punch on your heart and blood pressure (fast heartbeat, jitteriness, higher blood pressure), which can stack with side effects you may already feel on amitriptyline. Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
