Here's the gist for Paxil (paroxetine): your body relies on a liver enzyme called CYP2D6 to help clear this medicine out. Lab studies hint that yohimbine, the active part of yohimbe, can slow that enzyme down. If it does, paroxetine could build up a bit more than usual, which may mean stronger side effects like drowsiness, nausea, dizziness, dry mouth, or sweating.

I want to be honest about how strong this concern is: it's only based on test-tube research, not real people, so it's more theoretical than proven. In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the key thing to know if you're taking paroxetine (Paxil) and thinking about a yohimbe supplement. Paroxetine is a strong blocker of a liver enzyme called CYP2D6, which happens to be the same enzyme your body uses to break down yohimbine, the active compound in yohimbe. With that enzyme slowed down, yohimbine can't clear out as fast, so it builds up to higher levels in your blood.

In real terms, that means yohimbe's own side effects can hit harder: think raised blood pressure, a racing or pounding heart, jitteriness, and anxiety. That's especially worth pausing on since you may already be taking paroxetine for anxiety or panic. This isn't a guess, it's been seen in actual patients. Paroxetine decreases the clearance of yohimbine and may increase its effects.
Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine by about 350% and reduces the clearance of yohimbine by about 80% compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers. No significant changes in pharmacokinetic parameters of yohimbine were observed with coadministration of paroxetine in patients who are poor CYP2D6 metabolizers.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. What Is Paroxetine Used For?

Brand names: Paxil CR, Paxil, Brisdelle

Paroxetine tablets, film-coated tablets, and oral suspensions are approved in adults for major depressive disorder (MDD), obsessive compulsive disorder (OCD), panic disorder, social anxiety disorder, generalized anxiety disorder (GAD), and post-traumatic stress disorder (PTSD). The extended-release tablet forms (Paxil CR, paroxetine HCL CR, and paroxetine extended-release tablets) are approved in adults for MDD, panic disorder, social anxiety disorder, and premenstrual dysphoric disorder (PMDD).

Brisdelle (paroxetine 7.5 mg capsule) and generic paroxetine capsules are approved specifically for moderate to severe vasomotor symptoms (hot flashes) associated with menopause — not for any psychiatric condition. Paroxetine tablets are not approved for use in children.
