Theoretically, yohimbe might reduce the effects of antihypertensive drugs.
Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control.

Here's the simple version: you take diltiazem to relax your blood vessels and bring your blood pressure (and chest pain) under control. Yohimbe works in the opposite direction. The yohimbine in it can tighten blood vessels, nudge your blood pressure up, and speed up your heart rate. So adding yohimbe may partly cancel out the work diltiazem is doing, leaving your numbers higher than you and your doctor want.

To be fair, this concern is mostly theoretical, based on how yohimbine behaves rather than studies of this exact pairing. Diltiazem isn't a prodrug, so this is about tug-of-war on blood pressure, not how the drug is broken down. Over-the-counter yohimbe doses also vary a lot. Before trying it, please check with your pharmacist or doctor first.

Diltiazem also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the thing to keep an eye on with this pair: your body clears yohimbine (the active part of yohimbe) using an enzyme called CYP3A4. Diltiazem slows that same enzyme down, so in theory it could let yohimbine build up higher than expected and hang around longer.

Why that matters: yohimbe on its own can already push up your blood pressure and speed up your heart. That works directly against what diltiazem is trying to do, which is relax your vessels and calm your heart rate. More yohimbine in your system could mean a racing or pounding heartbeat, jitteriness, anxiety, or spikes in blood pressure. I'll be honest, this interaction is theoretical and based on how the drugs behave, not on strong real-world studies. Still, please check with your pharmacist or doctor before combining them.

Here's the gist for your diltiazem (Cardizem and similar brands). Diltiazem is cleared from your body by a liver enzyme called CYP3A4. Lab studies suggest yohimbe may speed that enzyme up, which in theory could clear diltiazem a little faster and make it slightly less effective. If that happened, you might notice your blood pressure creeping up or chest pain (angina) not being as well controlled.

The good news is this is only a theoretical concern based on test-tube research, and it's rated as minor. We don't have real-world cases showing a big problem. Still, given that yohimbe itself can raise blood pressure and speed up your heart, it's not a great match with a heart medication. Please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. What Is Diltiazem Used For?

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL

Oral extended-release forms of diltiazem — including Matzim LA, Cardizem LA, Cartia, Cartia XT, Tiadylt ER, and various generic capsules and tablets — are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain brought on by activity). Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).

Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
