Yohimbe and Caffeine (OTC drug)

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Yohimbe interacts with whole categories of medication. Caffeine (OTC drug) falls into 3 of those categories, so it can interact with Yohimbe in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

STIMULANT DRUGS

1 of 3

Why this applies to Caffeine (OTC drug)

Caffeine (OTC drug) falls under STIMULANT DRUGS — the group Yohimbe interacts with.

Theoretically, taking yohimbe with stimulant drugs can have additive effects.
Yohimbine, a constituent of yohimbe, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. Theoretically, taking yohimbe with stimulant drugs can have additive stimulant and hypertensive effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: both caffeine and yohimbe are stimulants, so taking them together can pile their effects on top of each other. Caffeine already revs up your heart and your nervous system, and yohimbine (the active part of yohimbe) does something similar by raising blood pressure and heart rate. Stack them and you may feel it.

What might that look like? A racing or pounding heartbeat, jitteriness, anxiety, trouble sleeping, and higher blood pressure. I'll be honest, this concern is theoretical, based on how these two work rather than strong studies, so it's not a guaranteed problem. Still, if you have heart issues or high blood pressure, please check with your pharmacist or doctor before combining them.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Caffeine (OTC drug) also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that yohimbe extract induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for caffeine pills like Vivarin: your liver uses an enzyme called CYP1A2 to break caffeine down. Lab studies suggest yohimbe may speed up that enzyme, which could mean your body clears caffeine a little faster than usual. In plain terms, your caffeine pick-me-up might feel weaker or wear off sooner.

The good news is this is a minor, theoretical concern based only on test-tube research, not real people, so I wouldn't lose sleep over it. The bigger issue is that yohimbe and caffeine are both stimulants. Together they can stack up jitteriness, a racing heart, anxiety, and higher blood pressure. If you take both, check in with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Here's the gist for caffeine pills like Vivarin. Lab studies suggest yohimbe may speed up a liver enzyme called CYP3A4, which is one of the enzymes that helps break down caffeine. In theory, that could clear caffeine a little faster and make its pick-me-up effect feel slightly weaker or shorter.

Honestly, though, this is a very minor concern. The evidence is only from test-tube and animal work, not real people, so the real-world impact is probably small. The bigger issue is that both yohimbe and caffeine are stimulants. Together they can stack up and cause a racing heart, jitteriness, anxiety, and higher blood pressure. If you're combining them, check with your pharmacist or doctor first.

About Yohimbe

What is Yohimbe?

Yohimbe
  • Erectile dysfunction
  • Low sex drive
  • Sexual performance
  • Weight loss
  • Athletic performance

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.
