Metronidazole falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Yohimbe interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the short version: this concern is really about the yohimbe, not the metronidazole (Flagyl). Your body uses an enzyme called CYP3A4 to help clear yohimbine, the active compound in yohimbe. Metronidazole can mildly slow that enzyme down. In theory, that could let yohimbine build up a bit more than usual, nudging up its side effects like a racing heartbeat, jitteriness, anxiety, or higher blood pressure.

I'll be honest, this is a theoretical interaction based on how the drugs work, not on strong real-world reports, so I don't want to alarm you. Metronidazole itself isn't changed here. Since yohimbe can be risky on its own, please run it by your pharmacist or doctor before combining the two.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Metronidazole treats three main types of infections. It's used for trichomoniasis (a sexually transmitted parasitic infection), amebiasis (an intestinal or liver infection caused by a parasite), and serious anaerobic bacterial infections — infections caused by bacteria that thrive without oxygen, which can affect the abdomen, skin, bones, lungs, and other areas.

Because sexual partners can pass trichomoniasis back and forth, both partners are typically treated at the same time, even if one has no symptoms.
