QT INTERVAL-PROLONGING DRUGS
Why this applies to Metronidazole
Metronidazole falls under QT INTERVAL-PROLONGING DRUGS — the group Arsenic interacts with.
Up to 40% of patients treated with prescription arsenic trioxide have a prolonged QT interval on their electrocardiogram (ECG). Theoretically, non-prescription arsenic could have an additive effect when combined with drugs that prolong the QT interval.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the concern with mixing arsenic and metronidazole (Flagyl). Prescription arsenic trioxide, used in some cancers, is well known to stretch out the heart's electrical cycle, something we call the QT interval. In studies, up to 40% of people on it showed this change on their ECG. Metronidazole can also nudge the QT interval in the same direction, so stacking the two could add up and, in rare cases, trigger a dangerous irregular heartbeat.
Metronidazole isn't a prodrug here, so this isn't about one activating the other. It's simply two things pulling on the heart's rhythm at once. The good news: the added risk from any non-prescription arsenic is theoretical, and you shouldn't be taking arsenic on your own anyway. Metronidazole treats infections caused by certain parasites and by bacteria that grow without oxygen (anaerobic bacteria). Which use applies depends on the product.
Oral products (tablets, capsules and Likmez suspension) are used for trichomoniasis, a sexually transmitted infection. Likmez is also used for amebiasis and for anaerobic bacterial infections in adults. Metronidazole injection treats serious anaerobic infections. Vaginal gels treat bacterial vaginosis. Topical cream and lotion treat rosacea.
