Yohimbe and Dipyridamole

Here is the interaction we found between these two, with the evidence behind it.

Minor

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Dipyridamole

Dipyridamole falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Yohimbe interacts with.

Theoretically, combining yohimbe bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for folks taking dipyridamole (Persantine) alongside yohimbe. Dipyridamole helps keep blood platelets from clumping together, which is exactly how it lowers your clot risk after a valve replacement. Yohimbine, the active piece of yohimbe, has shown in lab studies that it can also make platelets a little less sticky. So in theory, putting the two together could nudge your blood toward bleeding more easily, things like easy bruising, nosebleeds, or bleeding that takes longer to stop.

That said, this is a minor, theoretical concern. It's only been seen in test-tube work, never confirmed in real patients, and we don't even know how much yohimbine your body absorbs from the bark. I'd still mention it to your pharmacist or doctor, especially since yohimbe can raise blood pressure and heart rate on its own.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Dipyridamole page

“Dipyridamole (Persantine) keeps platelets from clumping so clots don't form, often after a stroke or heart valve surgery, and that job is exactly why the 219 supplements on this page warrant real vigilance. The bulk of the list, including most of the 190 moderate entries, adds its own blood-thinning or antiplatelet effect on top of the…”

About Yohimbe

What is Yohimbe?

Yohimbe
  • Erectile dysfunction
  • Low sex drive
  • Sexual performance
  • Weight loss
  • Athletic performance

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Dipyridamole

What Is Dipyridamole Used For?

Brand names: Persantine, Dipyridamole, Persantin, Persantin Injection, Persantin Retard

Dipyridamole tablets are used alongside warfarin (a blood thinner) to help prevent dangerous blood clots from forming after cardiac valve replacement surgery. It is not used alone for this purpose — it works as an add-on to anticoagulant therapy.

Dipyridamole Injection is used as an alternative to exercise during thallium myocardial perfusion imaging — a type of heart scan that checks blood flow through the coronary arteries. It is given to patients who are unable to exercise adequately for the test.
