Here's the gist: your body partly relies on an enzyme called CYP2D6 to clear duloxetine (Cymbalta) out of your system. Lab studies suggest yohimbine, the active piece of yohimbe, can slow that enzyme down. If it does, duloxetine could build up a bit higher than usual, which may bump up its typical side effects like nausea, dizziness, dry mouth, trouble sleeping, jitteriness, or a faster heartbeat.

I want to be honest about the evidence though: this is only seen in test-tube research, not in real people, so it's more of a theoretical concern. Still, yohimbe on its own can raise blood pressure and heart rate, and duloxetine can too, so the combo gives me pause. Please check with your pharmacist or doctor before pairing them.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the thing to know if you're thinking about pairing these two. Duloxetine acts as a brake on a liver enzyme called CYP2D6, and yohimbine (the active part of yohimbe) is cleared from your body by that very same enzyme. When duloxetine slows that pathway down, yohimbine can't break down as quickly, so it tends to build up to higher levels than it would on its own.

What does that mean for you in practice? The unwanted effects of yohimbe become more likely and more intense. We're talking spikes in blood pressure, a racing or pounding heart, anxiety, jitteriness, and trouble sleeping. Since over-the-counter yohimbe products are already unpredictable in strength, please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

What Is Duloxetine Used For?

Brand names: Cymbalta, Drizalma Sprinkle, Irenka

Duloxetine delayed-release capsules treat major depressive disorder (MDD) and generalized anxiety disorder (GAD) in adults. They also manage diabetic peripheral neuropathic pain (nerve pain from diabetes), fibromyalgia, and chronic musculoskeletal pain (such as chronic low back pain and pain from osteoarthritis) in adults.

GAD is also approved in pediatric patients 7 years and older for Cymbalta, Drizalma Sprinkle, and certain duloxetine delayed-release capsule products. Fibromyalgia is approved in patients 13 years and older for Cymbalta and some duloxetine delayed-release capsule products. Age-specific approvals vary by product — follow your prescriber's guidance on which product is right for your age group.
