Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the basic idea with these two together. Your liver uses an enzyme called CYP2D6 to help process propoxyphene. Yohimbine, the active part of yohimbe, may slow that enzyme down a bit. If propoxyphene clears more slowly, more of it can hang around in your body, which could mean stronger effects like drowsiness, dizziness, constipation, slowed breathing, or a foggy head.

I want to be honest about the strength of this, though. So far it comes only from lab (test tube) studies, not real people, so we can't say how big a deal it is in everyday life. On top of that, yohimbe itself can spike blood pressure and heart rate. Please check with your pharmacist or doctor before pairing them.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the key thing to know about this pair: your body relies on an enzyme called CYP2D6 to break down yohimbine, the active piece of yohimbe. Propoxyphene happens to block that same enzyme. So when you take them together, the yohimbine can't clear out as quickly and tends to build up in your system.

That matters because yohimbine on its own can already push your blood pressure up and make your heart race. With propoxyphene slowing its breakdown, you may feel more of those effects, things like a pounding or fast heartbeat, jitteriness, anxiety, or higher blood pressure. This is a moderate, fairly likely interaction, so please check with your pharmacist or doctor before combining them rather than guessing on your own.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

Brand names: Darvon, Prophene-65, Margesic, Davon

This three-ingredient combination — butalbital, acetaminophen, and caffeine — is used to relieve the symptoms of tension headache (also called muscle contraction headache). All supported products, including tablets, capsules, and oral solution, share this same approved use.

It is worth knowing that evidence for using this combination to treat repeated, frequent tension headaches over time is limited. Because butalbital can be habit-forming, extended or repeated use is not recommended.
