Erythromycin falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Yohimbe interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the thing to know about pairing these two. Your liver uses an enzyme called CYP3A4 to break down yohimbine, the active part of yohimbe. Erythromycin slows that same enzyme down, so the yohimbine could clear out of your body more slowly and build up higher than expected.

That matters because yohimbe already tends to push your blood pressure up and speed your heart rate. More of it hanging around could make those effects stronger, leaving you with a racing or pounding heart, jitteriness, anxiety, headache, or a spike in blood pressure. I'll be honest, this is a theoretical concern based on how the drug works, not something proven in lots of people. Still, since over-the-counter yohimbe is unpredictable to begin with, please run it by your pharmacist or doctor before combining them.

Here's the gist for this pairing: lab studies hint that yohimbe may speed up an enzyme in your body called CYP3A4, which is one of the workhorses that processes erythromycin. If that enzyme runs faster, your body could clear the antibiotic a little quicker, leaving slightly lower levels in your bloodstream. In theory, that might make erythromycin a bit less effective at knocking out the infection it was prescribed for.

I want to be honest about how soft this evidence is. It comes only from test-tube research, not from real patients, and it's rated as a minor interaction. So I wouldn't lose sleep over it, but since finishing an antibiotic course matters, run any yohimbe use past your pharmacist or doctor first.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Erythromycin

What Is Erythromycin Used For?

Brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, Eryc, Ery-Tab, Rythro Enca, Erythro E-C, Erythro Base, Ilosone, Ilotycin

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.

Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
