Nortriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the gist for these two together. Your body relies on an enzyme called CYP2D6 to clear nortriptyline out of your system. Yohimbe (specifically its active compound, yohimbine) appears to slow that enzyme down. When that pathway is blocked, nortriptyline can hang around longer and build up to higher levels, which may amplify its usual side effects like a dry mouth, constipation, drowsiness, dizziness on standing, and a faster or irregular heartbeat.

I want to be honest about the evidence, though: this is based only on lab (test tube) research, not real patients, so it's more of a theoretical concern than a proven one. There's also a separate worry: both yohimbe and nortriptyline can raise blood pressure and heart rate on their own. Please check with your pharmacist or doctor before combining them.

Nortriptyline also falls under TRICYCLIC ANTIDEPRESSANTS (TCAs).

Theoretically, taking yohimbe with TCAs can increase adverse effects.
A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor. The effects of yohimbe bark itself are unclear; oral yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

Here's the gist if you take nortriptyline (Pamelor) and are eyeing a yohimbe supplement. Yohimbe's active piece, yohimbine, revs up your body's stress and adrenaline system. Nortriptyline, a tricyclic antidepressant, already nudges that same system. Stack the two and you may dial things up too far, leading to anxiety, jitteriness, tremor (shaky hands), a racing heart, or higher blood pressure. This isn't about your antidepressant being weakened or strengthened in your liver; it's the two having overlapping stimulating effects.

Honestly, this is mostly theoretical, with one small study using IV yohimbine, and we don't know how much yohimbine your body absorbs from bark supplements. Still, given the heart and blood pressure risks, I'd check with your pharmacist or doctor before combining them.

Here's the gist for combining yohimbe with nortriptyline. Lab studies suggest yohimbe may speed up an enzyme called CYP1A2, which is one of the enzymes your body uses to break down nortriptyline. If that enzyme runs faster, your nortriptyline could clear out a bit quicker, potentially lowering its levels and making it work less well for your depression.

I want to be honest with you, though: this is only theoretical, based on test-tube research, not real people. The interaction is rated minor, so I wouldn't lose sleep over it. Still, yohimbe carries its own risks (raising blood pressure and heart rate), and nortriptyline can already affect your heart. Please loop in your pharmacist or doctor before adding it.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Nortriptyline is approved to relieve symptoms of depression. This applies to nortriptyline hydrochloride capsules, nortriptyline hydrochloride oral solution, and Pamelor capsules — all used for the same core indication.
