Here's the gist for these two. Your body uses an enzyme called CYP2D6 to help break down and clear clomipramine. Yohimbine, the active piece of yohimbe, may slow that enzyme down a bit. If clearing slows, clomipramine can build up in your system, and that could mean more of its usual side effects, like a dry mouth, constipation, drowsiness, dizziness on standing, or a faster heartbeat.

I want to be honest about the evidence here, though. This is only seen in lab (test tube) studies, not in real people, so it's more of a theoretical concern than a proven one. Still, yohimbe on its own can spike blood pressure and heart rate, so please check with your pharmacist or doctor before combining them.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the thing to keep in mind with this pairing: your body relies on a liver enzyme called CYP2D6 to clear out yohimbine, the active compound in yohimbe. Clomipramine (Anafranil) blocks that same enzyme, so it can act like a traffic jam, letting yohimbine pile up higher than expected and linger longer in your system.

When yohimbine builds up, its own side effects can get stronger, things like a racing heartbeat, spikes in blood pressure, jitteriness, and anxiety. That's a real concern here, since clomipramine itself can already speed up the heart and raise blood pressure. The two together may amplify each other. This is rated a moderate, fairly likely interaction. Please check with your pharmacist or doctor before combining them.

Clomipramine also falls under TRICYCLIC ANTIDEPRESSANTS (TCAs).

Theoretically, taking yohimbe with TCAs can increase adverse effects.
A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor. The effects of yohimbe bark itself are unclear; oral yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

Here's the gist: when you pair yohimbe with clomipramine, the worry isn't about your antidepressant losing strength. Instead, the two seem to stack up on each other in a way that can crank up nervous-system stimulation. In a small study, people already on a tricyclic who were given yohimbine ran into severe anxiety, agitation, and tremor. Clomipramine on its own can already bump up heart rate and blood pressure, so adding yohimbe (which does the same) may make those feelings worse.

I'll be honest, this is mostly theoretical and based on how the two work, not strong proof, and how much yohimbine your body actually absorbs from bark supplements is unclear. Still, with clomipramine's known effects, I'd run it past your pharmacist or doctor first.

Here's the gist. Your body uses an enzyme called CYP1A2 to help break down clomipramine. Lab studies (done in test tubes, not in people) suggest yohimbe might speed that enzyme up. If that happened in real life, your body could clear clomipramine a little faster, which might lower its levels and make it work less well for your OCD symptoms.

I want to keep this in perspective. This is a theoretical concern rated as minor, and we only have early lab data, not human studies. So I wouldn't lose sleep over it. Still, clomipramine takes weeks to settle in, and yohimbe carries its own risks like raising blood pressure and heart rate. Please check with your pharmacist or doctor before combining the two.

Here's the basic idea with yohimbe and clomipramine (Anafranil). Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
