Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control.

Here's the simple version: you take verapamil to relax your blood vessels and ease your blood pressure or chest pain. Yohimbe works in the opposite direction. The yohimbine in it can tighten blood vessels and push your blood pressure and heart rate up, so it may partly cancel out the calming work verapamil is doing.

In practice, that could mean less steady blood pressure control, more chest pain, or feeling your heart race. Keep in mind this warning is theoretical, based on how yohimbine acts rather than on cases of people on verapamil specifically. Still, because over-the-counter yohimbe doses vary a lot and can be strong, I'd check with your pharmacist or doctor before pairing the two. Don't change your verapamil on your own.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the thing with this pairing: verapamil slows down a liver enzyme called CYP3A4, and that same enzyme is what your body uses to break down yohimbine (the active part of yohimbe). So when you take them together, the yohimbine may clear out more slowly and build up higher than expected. That can amplify yohimbe's typical effects, like a racing heartbeat, jitteriness, anxiety, and spikes in blood pressure.

I'll be straight with you: this is based on how the drugs work on paper, not on strong human studies, so it's a theoretical concern. Still, yohimbe is unpredictable on its own, and pairing it with a blood pressure and heart medication is worth a quick chat with your pharmacist or doctor before you try it.

Here's the gist for these two together. Lab studies suggest yohimbe might speed up an enzyme called CYP1A2, which is one of the pathways your body uses to clear verapamil. If that enzyme runs faster, verapamil could be broken down a bit quicker, which in theory might leave you with slightly less of it. That could mean your blood pressure or chest pain (angina) isn't controlled quite as well as usual.

I want to be honest about the strength of this: it's theoretical, based only on test-tube research, and rated minor. We don't have real-world cases showing it causes problems. The bigger worry with yohimbe is its own habit of raising blood pressure and heart rate, which works against what verapamil is doing. Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
