Why this applies to Doxepin

Doxepin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the short version for these two together. Your body partly relies on a liver enzyme called CYP2D6 to break down doxepin. Lab studies suggest yohimbine (the active piece of yohimbe) can slow that enzyme down. If it does, doxepin may clear out more slowly and build up in your system, which could mean stronger effects like next-morning grogginess, dry mouth, dizziness, or constipation. Doxepin is not a prodrug, so more drug in the body means more effect, not less.

Keep in mind this is only test-tube evidence, so it's more of a theoretical concern than something proven in people. Still, yohimbe on its own can raise blood pressure and heart rate, so please check with your pharmacist or doctor before pairing them.

TRICYCLIC ANTIDEPRESSANTS (TCAs)

Doxepin also falls under TRICYCLIC ANTIDEPRESSANTS (TCAs).

Theoretically, taking yohimbe with TCAs can increase adverse effects.
A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor. The effects of yohimbe bark itself are unclear; oral yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

Here's the gist if you take doxepin (whether for sleep, depression, or anxiety) and you're eyeing a yohimbe supplement. Yohimbe's active compound, yohimbine, revs up your body's stress signaling by blocking certain adrenaline receptors. Pair that with a tricyclic like doxepin and the worry is an additive jolt to your nervous system.

In one small study, people on TCAs who got injected yohimbine ran into real trouble: bad anxiety, jitteriness, and tremor. That's stronger and more direct than swallowing bark, and we honestly don't know how much yohimbine you actually absorb from over-the-counter products. So this is more theoretical than proven for capsules, but yohimbe can already spike blood pressure and heart rate on its own. I'd run it by your pharmacist or doctor before combining the two.
