Quinidine falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS — the group Yohimbe interacts with.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the thing to know if you're on quinidine for your heart rhythm: quinidine is one of the strongest blockers of a liver enzyme called CYP2D6. Yohimbe's active ingredient, yohimbine, is normally cleared by that same enzyme. So when quinidine slows it down, yohimbine can build up in your body and hit you harder than expected.

That matters because yohimbine on its own can spike blood pressure, race your heart, and trigger jitteriness or anxiety. Stacked with a heart medication, that's exactly the kind of strain you don't want. This isn't yohimbe changing your quinidine, it's your quinidine making yohimbe more potent. The evidence is moderate but reasonable, so please check with your pharmacist or doctor before combining them.

Here's the idea behind this pairing. Quinidine is partly broken down by a liver enzyme called CYP3A4. Lab studies suggest yohimbe may speed that enzyme up, which could clear quinidine a little faster and slightly lower its levels. Since quinidine is an antiarrhythmic that keeps your heart rhythm steady, anything that weakens its effect could, in theory, let an irregular heartbeat slip back in.

I want to be honest about how thin this evidence is. It comes only from test-tube research, not from people, so it's labeled minor and theoretical. Still, quinidine is a drug where small changes matter, so don't add yohimbe without first checking with your pharmacist or doctor.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Quinidine

What Is Quinidine Used For?

Brand names: Quinaglute Duratabs, Quinaglute, Duraquin, Cardioquin, Cin-Quin, Quinora, Lin-Qin, Quinova, Quinidex Extentabs

Quinidine is used to help restore a normal heart rhythm in people with atrial fibrillation or atrial flutter (irregular, rapid heart rhythms originating in the upper chambers). It is also used on an ongoing basis to reduce how often those episodes come back, in patients whose episodes are frequent and poorly tolerated — but only after other options have been tried first.

Quinidine sulfate oral tablets and quinidine gluconate extended-release oral tablets are both approved for these heart-rhythm uses. Quinidine sulfate tablets are additionally used as part of an approved treatment regimen for life-threatening Plasmodium falciparum malaria, in combination with quinidine gluconate injection (the central component of that regimen). Both formulations may also be used to suppress recurrent, documented life-threatening ventricular arrhythmias (dangerous rapid rhythms in the lower chambers of the heart), such as sustained ventricular tachycardia.
