Diltiazem, Enalapril falls under ANTIHYPERTENSIVE DRUGS — the group Yohimbe interacts with.

Theoretically, yohimbe might reduce the effects of antihypertensive drugs.
Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control.

Teczem is a two-in-one blood pressure pill that combines diltiazem (a calcium channel blocker) with enalapril (an ACE inhibitor). Both are working to relax your blood vessels and keep your numbers down. Yohimbe pulls in the opposite direction. Its active piece, yohimbine, can rev up your nervous system and push blood pressure and heart rate up, which may partly cancel out the work your medicine is doing.

This is a theoretical concern based on how yohimbe behaves, not hard proof from people taking it with Teczem, so don't panic. Still, if you notice headaches, a racing heart, or higher readings at home, that's worth flagging. Please check with your pharmacist or doctor before adding yohimbe rather than changing anything on your own.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the thing to know about this combo. Your Teczem contains diltiazem, which slows down a liver enzyme called CYP3A4. That same enzyme is what your body uses to break down yohimbine, the active piece of yohimbe. So if you take them together, the yohimbine may clear out more slowly and build up in your system.

What does that mean in real life? More of yohimbine's typical effects, like a racing heart, jitteriness, anxiety, and spikes in blood pressure. That's a bit ironic, since you're taking diltiazem and enalapril to lower blood pressure in the first place. To be fair, this is a theoretical concern based on how the drugs work, not strong proof. Still, please check with your pharmacist or doctor before mixing them.

Here's the basic idea with Teczem (the diltiazem and enalapril combo). The diltiazem part is cleared by a liver enzyme called CYP3A4. In lab studies, yohimbe seems to speed up that enzyme, which could in theory lower diltiazem levels a bit and make it work less well. That might mean your blood pressure or heart rate isn't controlled as tightly as your doctor intended.

I want to be honest though: this is only test-tube evidence, it's rated minor, and we don't have real patient data showing it actually happens. The bigger worry is yohimbe itself, since it can raise blood pressure and speed up your heart, which works against this medicine. I'd skip yohimbe and check with your pharmacist or doctor first.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.
