Yohimbe and Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine

These two interact in 5 distinct ways. Each one is explained separately below.

Why Are There 5 Interactions?

Yohimbe interacts with whole categories of medication. Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls into 5 of those categories, so it can interact with Yohimbe in 5 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

1 of 5

Why this applies to Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine

Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

This is a combination product, and the piece I want to flag is the hydrocodone. Hydrocodone is a prodrug that your body partly converts through an enzyme called CYP2D6 into hydromorphone, which provides a good chunk of its pain relief. Lab studies suggest yohimbine (the active part of yohimbe) can block CYP2D6. If that happens in the body, hydrocodone might convert less efficiently, so you could get weaker pain control than expected.

Honestly, the bigger real-world worry is overlap of stimulant-type effects. Yohimbe can spike blood pressure and heart rate, and so can the caffeine and phenylephrine here, leaving you jittery or racing. This interaction is only theoretical, but please check with your pharmacist or doctor before combining them.

CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS

2 of 5

Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's the thing to know about this combination product (Hycomine Compound). One of its ingredients, chlorpheniramine, can gently block the liver enzyme called CYP2D6. As it happens, that's the same enzyme your body uses to clear yohimbine, the active compound in yohimbe. So when you take them together, the yohimbine may not break down as quickly and can build up higher than expected.

What does that look like in real life? Yohimbe on its own can already raise blood pressure, speed up your heart, and trigger jitteriness or anxiety. Higher yohimbine levels could make those effects stronger. On top of that, the phenylephrine and caffeine in this product are also stimulating to your heart and blood vessels, which can stack up. I'd recommend running this past your pharmacist or doctor before combining them.

STIMULANT DRUGS

3 of 5

Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine also falls under STIMULANT DRUGS.

Theoretically, taking yohimbe with stimulant drugs can have additive effects.
Yohimbine, a constituent of yohimbe, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. Theoretically, taking yohimbe with stimulant drugs can have additive stimulant and hypertensive effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to watch with this combo product: it contains two ingredients that already rev up your system. The caffeine is a stimulant, and the phenylephrine is a decongestant that can tighten blood vessels and nudge your blood pressure up. Yohimbe acts in a similar direction, so stacking them together could add up and push your blood pressure higher or make your heart race or pound.

In real life that might feel like jitteriness, anxiousness, a racing or pounding heartbeat, or a headache. I want to be honest that this concern is theoretical, based on how these things work rather than strong studies. Still, if you have any heart or blood pressure issues, please check with your pharmacist or doctor before pairing them.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

4 of 5

Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that yohimbe extract induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product (Hycomine Compound) has a lot going on, but the piece that matters here is the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2. Lab studies suggest yohimbe may speed that enzyme up a little, which in theory could lower caffeine levels and make it wear off faster. In plain terms, that's a small, mostly harmless effect, not something that puts your safety at risk.

I'd actually worry more about the other direction: yohimbe and caffeine together can both rev up your heart and blood pressure. Add the phenylephrine (a decongestant) and you could feel jittery, fast-hearted, or notice a higher blood pressure reading. The hydrocodone, chlorpheniramine, and acetaminophen aren't really part of this enzyme issue. Please check with your pharmacist or doctor before combining these.

Here's the gist with this combo product (Hycomine Compound), which packs in hydrocodone, acetaminophen, caffeine, chlorpheniramine, and phenylephrine. Lab studies hint that yohimbe might speed up a liver enzyme called CYP3A4, and that's the pathway that helps clear several of these ingredients. In theory, faster clearance could mean a slightly weaker effect, like a little less cough and pain relief from the hydrocodone.

I want to be honest with you: this is rated minor and only comes from test-tube research, not real people, so I wouldn't lose sleep over it. The bigger real-world concern is that yohimbe and the caffeine and phenylephrine in this product can all raise blood pressure and heart rate. Please check with your pharmacist or doctor before pairing them.

About Yohimbe

What is Yohimbe?

Yohimbe
  • Erectile dysfunction
  • Low sex drive
  • Sexual performance
  • Weight loss
  • Athletic performance

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.
