Here's the simple version. Nortriptyline is a tricyclic antidepressant, and along with lifting mood it has a side of what we call anticholinergic effects, things like dry mouth, constipation, blurry vision, and trouble urinating. Calabar bean contains physostigmine, which works in the opposite direction by boosting acetylcholine. So in theory it could blunt or partially cancel out nortriptyline's anticholinergic activity.

The good news: this is a theoretical concern based on how the chemicals behave, not something well documented in real patients, and it's rated minor and unlikely. The bigger issue is that Calabar bean itself is genuinely poisonous and should never be used as a supplement. Please don't change your nortriptyline, and talk to your pharmacist or doctor before taking anything new.
