Here's the basic idea. Clomipramine has some anticholinergic activity, meaning it dampens a nerve signal called acetylcholine. That's part of why it can cause dry mouth, constipation, blurry vision, and a faster heartbeat. Calabar bean works in the opposite direction. Its main compound, physostigmine, blocks the enzyme that clears acetylcholine, so that signal builds back up. In theory, the two could partly cancel each other out, slightly easing clomipramine's anticholinergic side effects.

I want to be honest with you though: this is a theoretical concern based on pharmacology, rated minor and unlikely. The bigger issue is that Calabar bean itself is highly poisonous and is not a safe supplement to take at all. Please don't use it, and talk with your pharmacist or doctor before combining anything new with clomipramine.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
