Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control.

Hyzaar combines two blood pressure medicines: losartan (an ARB) and hydrochlorothiazide (a water pill). The whole point of taking it is to keep your blood pressure in a healthy range. Yohimbe works in the opposite direction. Its active compound, yohimbine, can stimulate your nervous system and actually push blood pressure and heart rate up.

So putting these together may work against each other and make it harder for your Hyzaar to control your numbers. You could see higher readings, a faster or pounding heartbeat, or feel jittery and anxious. This concern is mostly theoretical, based on how yohimbine behaves, but it's a real enough worry that I'd check with your doctor or pharmacist before adding any yohimbe supplement.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.

Here's the practical picture for your Hyzaar (losartan plus hydrochlorothiazide). The lab-based concern is that yohimbe might speed up an enzyme called CYP3A4, which could in theory lower levels of drugs handled by it. With losartan there's a small twist: it's partly a prodrug that the liver converts into a more active form, so the net effect on blood pressure control is hard to predict, and the hydrochlorothiazide piece isn't really driven by this enzyme at all.

Honestly, this is based only on test-tube data, so it's a weak, theoretical interaction. The bigger real-world worry is that yohimbe itself can raise blood pressure and speed up your heart, which works against why you take Hyzaar. Please check with your pharmacist or doctor before adding it.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.
