Bosentan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the short version on pairing milk thistle with bosentan (Tracleer). Your body clears bosentan partly through a liver enzyme called CYP2C9. There's a theory that milk thistle might slow that enzyme down a little. If it did, bosentan could build up slightly, which in theory could mean more of its usual side effects like headache, flushing, swelling in your legs, or changes in liver tests.

The honest truth is the evidence here is weak and conflicting. Some lab studies hint at this, but better human studies suggest milk thistle doesn't meaningfully block this enzyme. So a real-world problem is unlikely. Since bosentan already needs liver monitoring, it's still worth a quick chat with your pharmacist or doctor before adding milk thistle. Don't change your dose on your own.

While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: your body uses a liver enzyme called CYP3A4 to clear bosentan (Tracleer), and there's a theory that milk thistle's main compound, silymarin, might slow that enzyme down. If that happened, more bosentan could linger in your system, which in theory could mean more of its usual side effects, like headache, flushing, swelling in the legs, or changes in your liver tests.

The good news is the evidence here is weak. This concern comes mostly from lab and animal studies, and human studies on other CYP3A4 drugs didn't show a real effect. So a meaningful problem is unlikely. Still, bosentan already gets close liver monitoring, so it's worth a quick word with your pharmacist or doctor before adding milk thistle.

Bosentan also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Your bosentan (Tracleer) relies on transporters called OATPs to get absorbed and taken up by your liver. Milk thistle may gently block one of these transporters, OATP-B1. In theory, that could mean a little less bosentan gets into your body, which might slightly soften how well it works to ease the high pressure in your lung blood vessels.

Here's the honest part: this is rated a minor concern, and the evidence comes only from lab studies, not real-world testing in people on bosentan. So the risk is mostly theoretical. Still, since bosentan helps you breathe and stay active, it's worth a quick chat with your pharmacist or doctor before adding milk thistle, rather than starting or stopping anything on your own.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
