Brentuximab Vedotin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: brentuximab vedotin (Adcetris) releases a small drug payload called MMAE, which your liver breaks down using an enzyme known as CYP3A4. Milk thistle might slow that enzyme down a little, and if it did, more of that payload could linger in your body. In theory that could mean a bit more of the usual side effects you'd watch for with this medicine, like numbness or tingling in your hands and feet, lower blood counts, fatigue, or nausea.

That said, the actual evidence here is thin. Lab results are mixed, and good human studies didn't show milk thistle meaningfully changing how similar drugs are cleared. A handful of case reports raised a flag with other cancer drugs, so it's worth a quick chat with your oncology team before adding milk thistle. Please don't change anything on your own.

About Brentuximab Vedotin Injection

What Is Brentuximab Vedotin Injection Used For?

Brand name: Adcetris

Adcetris (brentuximab vedotin) is used to treat classical Hodgkin lymphoma (cHL) in adults — including newly diagnosed advanced-stage disease, as consolidation therapy after a stem cell transplant, and in relapsed or refractory cases. It is also approved for pediatric patients aged 2 and older with previously untreated high-risk classical Hodgkin lymphoma, given in combination with chemotherapy.

Adcetris is also used for several T-cell lymphomas — including systemic anaplastic large cell lymphoma (sALCL), peripheral T-cell lymphomas (PTCL), primary cutaneous anaplastic large cell lymphoma (pcALCL), and CD30-expressing mycosis fungoides (MF). Additionally, Adcetris is used in combination with lenalidomide and a rituximab product for adults with relapsed or refractory large B-cell lymphoma (LBCL) who are not candidates for a stem cell transplant or CAR T-cell therapy.
