Losartan Potassium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the wrinkle with losartan: it's actually a prodrug. Your body uses an enzyme called CYP2C9 to turn losartan into its more powerful active form, which does most of the blood pressure lowering work. Milk thistle might slow that same enzyme down, which in theory could mean less of the active drug gets made and your blood pressure medicine works a little less well.

The good news is the evidence here is shaky and conflicting. Lab studies hint at a slowdown, but actual human studies haven't shown milk thistle meaningfully blocking CYP2C9. So a real-world problem is unlikely. Still, it's smart to mention milk thistle to your pharmacist or doctor, especially if your blood pressure readings start creeping up.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for losartan (Cozaar). One of the ways your body handles losartan involves a liver enzyme called CYP3A4. Milk thistle might slow that enzyme down a little, but honestly the research is all over the place. Good human studies show milk thistle does not meaningfully change how the body processes similar CYP3A4 medicines.

There's a small wrinkle worth knowing: losartan is actually a prodrug, meaning your liver converts it into its more powerful active form. So in theory, blocking that enzyme could blunt some of losartan's blood-pressure-lowering effect, not boost it. In real life this concern is very minor and unlikely. Still, if you start milk thistle, it's smart to keep an eye on your blood pressure and check in with your pharmacist or doctor.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
